REAL Transition Partners

Community Based Organization Mini Grant Awards Up to $2,500

REAL Transition Partners, funded by the Rehabilitative Services Administration, spans 13 states and territories. This includes Connecticut, Delaware, Washington D.C., Maine, Maryland, Massachusetts, New Hampshire, New Jersey, New York, Pennsylvania, Puerto Rico, Rhode Island, Vermont, and the Virgin Islands. Our goal is to increase knowledge, skills, and confidence of parents and youth with disabilities on transition from school to post-high school education, competitive employment, and independent living, by providing information, training and support.

We primarily focus our funding on community-based organizations in underserved areas. Successful projects should aim for financial stability, employment, or education. These are key for transitioning to adulthood.

Through REAL Transition Partners, local projects can get up to $2,500 each, with a total cap of funding three organizations. Grants are open from February 1 to September 30, 2025. Projects must finish by September 30, 2025, and reports are due by October 31, 2025.
