Parent Network of WNY’s healthcare passport is a valuable tool that provides the opportunity for improved communication as well as enhancing quality and continuity of care among healthcare providers and other members of a person’s care team.  It allows the family to give each of their child’s many providers a ‘snapshot’ the whole child including:

• How they like to be called
• What works/helps and what doesn’t
• Sensory needs
• How to make them feel comfortable
• Strategies for success

Providing this type of passport will present their unique needs in a succinct way that will ultimately optimize their care.
